Welcome to the HEAT!All the forms you'll need to register are listed below. 2009-2010 REGISTRATION FORMSThe registration packet
includes a lot of information and a handy checklist to be sure that you are up to speed on the changes.
The packet also contains all of the forms that need to be completed: Registration Form, Medical History Form, Application Addendum, AEA Automatic Withdrawal Forms;
and if applicable, Club Transfer Form, and Masters Registration Form. If more than one family member is joining the team, you will need an additional
Medical History Form for each swimmer. OUTREACHFamilies that currently qualify for reduced/free lunches at school will also qualify for USA Swimming's
Outreach Program. Outreach swimmers pay a reduced Annual Membership Fee. CHANGES FROM ACTIVE STATUSIf you plan to change your status with the club, you will need to read and complete the
Change in Status Form to avoid any unexpected financial obligations.
